Special thanks to Joan Kanady for her tireless assistance in optically scanning, typing and editing the forms that went into this volume and printing and copying numerous drafts of the manuscript. We also gratefully acknowledge the assistance of Noah J. Gordon, Esq., our editor, and the creative staff at Prentice-Hall Law and Business, and Lisa Nielsen of Borland International for a beautiful jacket design.

We also must thank the entire Kohn family, including Ted and Joanne Kohn, and their children Beth and Todd, for their continuous encouragement and, most especially, Lori, Katie and Joey for their patience and unwavering love and support during this prolonged endeavor. Above all, we wish to acknowledge the encouragement, support, and wisdom of the person to whom this book we lovingly dedicate.

Now, let's get on with the show. . . .
